The School recommends the following two students for participation in the 1999-00 Peachtree-Atlanta Kiwanis Rising Leaders Initiative. These students meet the criteria for Most Improved Students (see separate criteria list) based on the evaluation of the school counselor and/or principal (please attach individual evaluation forms for each student).

The School elects participation as follows (check 1 box)

	The School will (i) select 2 Most Improved Students, (ii) provide a person as a contact point for the program, (iii) assist the students in their commitment to attend 4 leadership training sessions, (iv) permit early dismissal for students attending training sessions (that start at 2:30 p.m.), and (v) forward all information on Rising Leaders to students in timely manner.
	The School chooses not to participate for 1999-00. (Most Improved Students will have a certificate mailed to them and will not be invited to the award luncheon or the training sessions.)
